One of the most common questions raised in ADHD peer support groups is surprisingly simple:

Why can I do something one day and not the next?

Many people with ADHD describe caring deeply about something, wanting to do it, and fully intending to act. Yet somehow the action never quite happens. Then, on another day, the same task feels manageable.

The inconsistency itself is often more frustrating than the difficulty.

People describe being able to manage jobs, families and major responsibilities while simultaneously struggling to reply to an email, make a phone call, start a task they genuinely want to complete, or have a conversation they know needs to happen.

Others talk about wanting to understand themselves better but avoiding information that feels emotionally uncomfortable. Some describe wanting more social connection while becoming increasingly isolated. Others find themselves postponing important decisions despite knowing those decisions matter.

What makes this difficult is that it rarely feels predictable.

Many people do not say:

“I can’t do this.”

They say:

“I can do it sometimes, but I don’t understand why.”

Simple explanations rarely explain the full picture

When people first learn about ADHD, they often hear that it is caused by low dopamine.

That idea exists for a reason.

Researchers observed that stimulant medications often improved attention and behaviour. Later work showed that many of these medications affect dopamine and noradrenaline systems in the brain. Over time, a simple explanation emerged: ADHD must be caused by a shortage of dopamine.

The problem is that the story became much simpler than the science.

Research increasingly suggests that ADHD cannot be adequately explained by dopamine levels alone. (Volkow et al., 2011; Dopamine dysfunction is not the main cause of ADHD, Cambridge University, 2024)

This matters because a simple dopamine-deficit explanation struggles to account for one of the most common ADHD experiences: inconsistency.

If the problem were simply a shortage of dopamine, it would be harder to explain why the same person can sometimes focus intensely, solve complex problems, navigate highly demanding situations, or engage deeply in something meaningful, yet struggle to begin a straightforward task or conversation on another day.

What dopamine actually does

Dopamine is a neurotransmitter, a chemical messenger that helps different parts of the brain communicate.

It plays a role in attention, learning, effort allocation and helping the brain decide what feels important.

But neurotransmitters do not work in isolation.

Timing, coordination and communication between systems matter as much as quantity.

Recent evidence suggests that ADHD may be less about how much dopamine is present and more about how reliably brain systems support attention, effort and behavioural regulation across changing situations. (Dopamine Signalling Dysregulation in ADHD, 2024)

The issue may not be whether intention exists.

The issue may be how consistently someone can turn intention into action.

This distinction matters because many people with ADHD care deeply about outcomes. They often know exactly what they want to do. What feels less reliable is their ability to consistently access and sustain the effort needed to follow through.

Why context matters so much

One reason ADHD can feel confusing is that performance often changes depending on circumstances.

Many people notice that engagement becomes easier when there is:

  • urgency
  • novelty
  • personal interest
  • accountability
  • clear expectations
  • emotional safety

The same person who cannot begin a task alone may complete it easily when someone else is depending on them.

The same person who avoids making a difficult phone call may have no difficulty supporting a friend through a crisis.

The same person who struggles to organise their own affairs may spend hours helping someone else solve a complex problem.

This does not mean people care only when something is exciting.

It suggests that attention, effort and action are heavily influenced by context.

The role of emotional safety is particularly important. Many decisions are not purely practical. They involve uncertainty, potential criticism, fear of getting things wrong, or anticipating how others might respond. These factors can shape whether a task feels approachable or overwhelming.

It’s not just dopamine

Dopamine remains important, but it is only one part of a much larger picture.

Research points towards the involvement of multiple interacting neurotransmitter systems, including noradrenaline, alongside broader differences in networks involved in executive functioning and self-regulation. Rather than searching for a single missing ingredient, researchers are increasingly examining how these systems work together.

This helps explain why no single explanation fully captures the ADHD experience.

A simple way to picture it

One way to think about this is the postman analogy.

If neurotransmitters deliver messages throughout the brain, ADHD is not necessarily a lack of letters.

It is more like a delivery system that is sometimes unreliable.

Some days everything arrives where it needs to go.

Other days messages are delayed, interrupted or arrive at the wrong time.

Sleep, stress, emotional demands, environment and competing priorities can all influence how smoothly those deliveries happen.

The result is an experience that feels inconsistent from the inside, even when effort and intention are present.

A note on the limits of research

Research has limits.

Neurotransmitters cannot be measured cleanly in real time, and much of what we know comes from indirect methods. Brains also do not behave the same way in controlled research settings as they do during everyday life, particularly when stress, fatigue, relationships and competing demands are involved.

These studies do not provide a complete explanation for why ADHD feels the way it does.

What they offer is a more useful framework for understanding why ADHD can be variable, context-dependent and difficult to predict.

What this means

Perhaps the most important thing to understand is that inconsistency does not mean the effort is not real.

Many people with ADHD care deeply about outcomes. They want to follow through. They want to be reliable. They want to understand themselves and be understood by others.

Research does not yet provide a complete explanation for why ADHD feels so inconsistent.

What it does suggest is that simple stories about laziness, lack of effort, or even “low dopamine” alone do not adequately explain the lived reality.

Perhaps that is why one of the most common ADHD questions remains:

“Why could I do it yesterday, but not today?”
